Registration
According to the New Jersey Department of Health and Senior Services, the following age appropriate vaccinations are required for students, per NJ Sanitary Code, Chapter 14, Immunization of Pupils in Schools (NJAC Title 8:5-4):

  • 18 months- 4 years of age:  4 doses DTaP; 3 doses Polio; 1 dose MMR; 1 dose Hib; 1 dose Varicella; 1 dose PCV; and 1 dose seasonal influenza (dose to be given between September 1 and December 31 of each year).
  • 5 years of age (Kindergarten entrants):  4 doses DTaP, with one dose given on or after the fourth birthday, or any 5 doses; 3 doses Polio, with one dose given on or after the fourth birthday, or any 4 doses; 2 doses MMR on or after the first birthday; 1 dose Varicella on or after the first birthday; and 3 doses Hepatitis B.
  • 11 years of age (Incoming 6th Graders):  TDaP booster and Meningococcal Vaccine.

Failure to meet the minimum vaccination requirements may result in your child being excluded from attending school.
